I do hereby declare that the personal data and information I provide is done so on a voluntary basis and I understand that the data will be used by Pfizer Limited and its third party vendors for the purpose of the campaign to raise awareness of AMR.
I do hereby consent to the use by Pfizer Limited of my personal information such as my name, email, occupation, image, video, voice, and / or all of them, in the item described above. In addition, I waive any right to inspect or approve the finished video recording.
I do hereby consent that the video pledge is the sole property of Pfizer Limited and I do not have any claim, right or stake in the video pledge submitted by me to Pfizer Limited.
I do hereby consent to the further disclosure of the information submitted by me to third parties with whom Pfizer Limited has entered into any alliance, partnership or other business arrangement with for the purpose of this campaign. I further consent to such third party being given access to personal information including my email, name, and video recording for the sole purpose of editing, preparing and publishing the same on the website of Pfizer Limited and maintaining records as necessary.
I do hereby acknowledge that Pfizer Limited will be using my personal information for making an application to third party vendors for the purposes of awards and recognition with a view to expand outreach of the campaign to raise awareness on AMR. For the purpose of such application(s), Pfizer Limited will also collect details of the date and time stamp of the video and my location at the time of recording of the video pledge.
I do hereby consent to the access to my personal information being accessed and used by third parties engaged by Pfizer Limited to facilitate the campaign, including, but not limited to making an application for the purposes of awards and recognition. I do hereby agree and acknowledge that any record awarded for the campaign and/ or any part thereof including the video pledges will solely belong to Pfizer and I have no claim or stake in such record.
